Eligibility Requirements

To qualify for the DOST Scholarship, applicants must:

  • Be a natural-born Filipino citizen
  • He or She must be a Regular second year college student enrolled in a DOST-SEI priority S&T course at identified universities in the Second Semester of the school year.
  • Must have a General Weighted Average, GWA of at least 83% or its equivalent and with no conditional or failing mark from the First Semester of First Year until the First Semester of Second Year in college, including in the midyear terms, if enrolled.
  • He or She must be of good moral character and in good health.
  • The candidate may have:
    • Applied for/taken the Junior Level Science Scholarship Examination but DID NOT QUALIFY for the scholarship.
    • Qualified for the Junior Level Science Scholarship but DID NOT AVAIL of the scholarship award.

  1. Form C – Certificate of Good Moral Character
  2. Form D – Certificate of Good Health
  3. Form E – Certificate of Program of Study and Year Level
  4. Form F – Certificate of Residency
  5. Form G – Commitment to Return Service
    • Form G1 – Merit/ RA 7687
    • Form G2 – RA 10612
  6. Form H – Applicant’s Certification Re: Not a DOST Scholar prior to this Application
  7. Form I – Applicant/Parent Declaration
  8. Official Transcript of Records (TOR) or True Copy of Grades (TCG) or Certified Grade Report downloaded from the school’s portal for First Semester of First Year until the First Semester of Second Year in college, including in the midyear terms, if enrolled.
  9. Applicant’s Recent picture – passport size (4.5 cm x 3.5 cm or 1.8 inches x 1.4 inches)
  10. Birth Certificate
  11. Proof of Income – Parent/s or Legal Guardian’s or Spouse’s 2024 Income Tax Return / BIR Form 1701 / Employment Contract for OFW / BIR Certificate of Exemption from Filing of ITR / Certificate of Indigency / Certificate of Employment with Compensation / Proof of Pension
  12. Notarized Affidavit of Guardianship (if with legal guardian)
  13. Marriage Certificate (if married)
